Please fill out this form to indicate your child's involvement in Family Ministry. There are opportunities for all ages. Sunday School for children ages 3-9 starts September 14th during the 10:00 a.m. service. This form will serve both as registration for Sunday School and involvment in family ministry. I give full permission for St. Stephen's Episcopal Church to use my child's likeness in a photograph or video on any and all publications, including website entries, without payment or any other consideration. In addition, I waive the right to inspect or approve the finished product, including written or electronic copy, wherein their likeness appears. I waive any right to royalties or other compensation arising or related to the use of such photograph or video.

Children under three are not yet old enough for Godly Play.  We do offer nursery care every fourth Sunday during the 10 a.m. worship service.  Please indicate your intentions for your under 3 year old during church below.

Children 3-9 will be enrolled in Godly Play. Placements in classes will be determined by using the information in this registration form as well as in conversation with parents
